The Passport Office at the British Embassy, Washington DC, is the only place in the USA that issues British passports (except in emergencies):

The phone line is open Monday to Friday from 8:30 a.m. to 8:30 p.m. Eastern Standard/Daylight Time, excluding public holidays.

Visas: A national of the United States does not need a visa to come to the UK as a tourist or visitor. On arrival in the UK you must satisfy an immigration officer that you qualify for entry to the United Kingdom as a visitor under the Immigration Rules.
